Benitec Reports All Nine Treated Patients Responded to BB-301 Gene Therapy in Rare Muscle Disease Study

Interim Phase 1b/2a results showed durable swallowing improvements at the low dose and larger three-month gains at the high dose in oculopharyngeal muscular dystrophy

HAYWARD, California, October 28, 2026. Benitec Biopharma reported interim results from the first nine patients treated with BB-301 in an ongoing Phase 1b/2a study for oculopharyngeal muscular dystrophy. The company said every treated patient met its formal responder definition, with durable improvements in the six-person low-dose cohort and larger early changes in the three-person high-dose cohort.

BB-301 is an investigational gene therapy delivered directly to pharyngeal muscles. It is designed to silence expression of the mutant PABPN1 gene while supplying a replacement version intended to support normal protein function. The study focuses on dysphagia, a progressive swallowing impairment that can lead to aspiration, malnutrition and reduced quality of life.

What the interim update showed

Benitec reported that high-dose patients showed greater relative improvement at three months in throat-emptying ability, measured by videofluoroscopic assessment of food and liquid remaining after a swallow, and in functional swallowing capacity measured by the Cold-Water Timed Drinking Test. Low-dose patients continued to show durable changes during longer follow-up.

The phrase 100 percent responders is striking but needs its denominator: nine people. In a small, uncontrolled study, one additional non-responder would materially change the percentage. The formal response definition, baseline variability, evaluator blinding and repeatability of the swallowing tests are therefore as important as the headline rate.

Why objective swallowing measures matter

Oculopharyngeal muscular dystrophy progresses slowly, and swallowing performance can vary with food consistency, fatigue, coaching and test conditions. Videofluoroscopy provides an anatomical and functional view, while timed drinking offers a practical measure of capacity. Using more than one endpoint can strengthen interpretation when the changes move in the same direction.

Future updates should provide patient-level trajectories, absolute as well as percentage changes, confidence intervals, prespecified responder thresholds and the relationship between laboratory measures and daily eating. Patient-reported choking, meal duration, aspiration events, weight and feeding support would help show whether test improvements translate into meaningful life changes.

Durability is the central gene-therapy question

A local gene therapy must produce sustained benefit to justify an invasive administration procedure and long-term follow-up. The low-dose cohort now provides more mature observation than the high-dose cohort, but comparisons across dose groups are not randomized. Differences may reflect baseline disease, injection coverage, follow-up or individual anatomy rather than dose alone.

The therapy combines RNA interference with gene replacement, which creates several technical dependencies. Vector distribution must reach enough relevant muscle fibers, silencing must reduce mutant protein without excessive off-target effects, and replacement expression must remain within a useful range. Manufacturing consistency and injection technique could influence outcome across treatment centers.

Safety and next steps

The full presentation should detail procedure-related events, immune responses, laboratory abnormalities, serious adverse events and any evidence of local inflammation or weakness. Because the treatment is intended to have a lasting effect, surveillance should extend beyond the period in which swallowing changes are first measured.

Benitec has previously described plans for a pivotal study and regulatory discussions. Interim activity can inform dose selection and endpoint design, but it does not establish effectiveness. A later trial will need a control strategy that accounts for slow progression and the ethics of sham or delayed treatment, together with centralized assessment of swallowing outcomes.

Why it matters

The update provides a rare human dataset for a disease with no approved therapy that corrects its underlying genetic mechanism. Consistent responses across two doses support continued development, while the small sample and uncontrolled design require restraint. The program will create value only if the functional changes remain durable and reproducible in a larger population.

The next checkpoints are longer high-dose follow-up, complete safety, a transparent responder definition and agreement with regulators on a pivotal protocol. BB-301 remains investigational, and the current results should not be interpreted as proof that every eligible patient will benefit.
